110-300-0165(2)(d) Licensee had a plastic bag under bathroom sink accessible to the children. 110-300-0165(3)(f) Licensee has a large 6 cube shelf that is not securely anchored to the wall causing a tipping hazhazard. 110-300-0165(4)(g) Licensee has a large tree house platform deck with several stairs accessbile to the children in the licesned back yard that has more then 18inches of drop zone.
